Helps eligible persons remain in their homes in the community.Once eligible participant and or representative will choose an approved Supports Planning Agency to guide them through developing a plan for services. A registered nurse does an assessment and monitors for quality assurance and safety. The three available programs are Community First Choice, Community Options, and Community Personal Assistance.
